Pretreatment SUVmax value to predict outcome in patients with stage III NSCLC receiving concurrent chemoradiotherapy

INTRODUCTION: Stage III disease accounts for approximately one-fourth of all non-metastatic non-small cell lung cancer (NSCLC). The patients who are not candidates for curative resection are offered concomitant chemoradiotherapy. In this subgroup, which is difficult to manage, studies that address the role of PET-CT to predict outcome measures specifically for stage III NSCLC receiving concurrent chemoradiotherapy may help better risk stratification. This study aimed to assess whether baseline PET maximum standardized uptake value (SUVmax) value in stage III NSCLC treated with concurrent chemoradiotherapy would independently identify patients with high risk of progression and death.
METHODS: The study population consisted of patients aged 18 years or more with unresectable stage III histologically or cytologically proven NSCLC who received concurrent chemoradiotherapy. From 2007 to 2014, medical records of patients admitted to our institution were retrospectively analyzed. Pretreatment PET-CT SUVmax values were recorded for each patient. These values were categorized as low or high according to the median SUVmax measure of the study population.
RESULTS: A total of 175 patients were analyzed. The median follow-up time was 23 months (range 6–109). The PET-CT SUVmax values ranged from 3.5 to 46 with a median value of 14. The median overall survival was 25 months in SUVmax <14 and 18 months in SUVmax ≥14 group (p=0.023). The median progression-free survival was 16 months in SUVmax <14 and 11 months in SUVmax ≥14 group (p=0.033). Multivariate analysis revealed that both PET-CT SUVmax value (p<0.001) and age (p=0.016) were independent significant predictors for overall survival (OS).
DISCUSSION AND CONCLUSION: The results of this study involving patients with stage III NSCLC receiving concurrent chemoradiotherapy provide evidence that suggests that high values of pretreatment SUVmax, an indicator of metabolic tumor burden, predicted a higher risk of disease progression and death.

Eşzamanlı Kemoradyoterapi Uygulanan Evre III Küçük Hücreli-Dışı Akciğer Kanserinde Başlangıç PET-CT Suv max Değerinin Klinik Sonlanım Noktalarına Etkisi

GİRİŞ ve AMAÇ: Evre III hastalık tüm Küçük Hücreli-Dışı Akciğer Kanseri (KHDAK) 'nin yaklaşık dörtte birini oluşturur. Birçok hasta küratif rezeksiyon için aday değildir ve bu hastalara eş zamanlı kemoradyoterapi önerilir. Eş zamanlı kemoradyoterapi uygulanan evre III KHDAK hastalarına spesifik olarak klinik sonuçları predikte ettirmede PET-BT'nin rolünü ele alan çalışmalar, yönetilmesi zor olan bu alt grupda daha iyi risk katmanlaşmasına yardımcı olabilir.
Amaç: Bu çalışmanın amacı, eşzamanlı kemoradyoterapi ile tedavi edilen evre III KHDAK'de başlangıç PET SUV max değerinin, relaps ve ölüm riski yüksek olan hastaları bağımsız olarak belirleyip belirlemediğini değerlendirmektir.
Çalışma Tasarımı: Çalışma popülasyonu, 18 yaş ve üstü, eşzamanlı kemoradyoterapi uygulanan, histolojik veya sitolojik olarak kanıtlanmış evre III KHDAK tanılı hastalardan oluşmaktadır.


YÖNTEM ve GEREÇLER: 2007'den 2014'e kadar kurumumuza başvuran hastaların tıbbi kayıtları retrospektif olarak incelendi. Her hasta için tedavi öncesi PET-CT SUV max değerleri kaydedildi ve çalışma popülasyonunun medyan SUV maks ölçümüne göre düşük veya yüksek olarak kategorize edildi.
BULGULAR: Toplam 175 hasta analiz edildi. Ortanca takip süresi 23 aydı (6-109). PET-CT SUV max değerleri 3.5 ile 46 arasında değişmekte olup medyan değer 14 dü. Medyan genel sağkalım SUV max <14 olan hastalarda 25 ay iken SUV max ≥14 olanlarda 18 aydı (p = 0.023). Medyan progresyonsuz sağkalım SUV max <14 saptanan hastalarda 16 ay, SUV maks ≥14 olanlarda ise 11 aydı (p = 0.033). Multivariate analiz, hem tedavi öncesi PET-CT SUV max değerinin (p <0.001) hem de yaşın (p = 0.016) genel sağkalım için bağımsız birer belirteç olduğunu ortaya koydu.
TARTIŞMA ve SONUÇ: Eş zamanlı kemoradyoterapi alan evre III KHDAK hastalarını kapsayan bu çalışmanın sonuçları, metabolik tümör yükünün bir göstergesi olan tedavi öncesi yüksek SUVmax değerlerinin hastalık progresyonu ve ölüm açısından yüksek riski predikte ettirdiğini düşündürmektedir.
